The Colte Partnership are currently recruiting for an exciting new position of Advanced Nurse Practitioner – central telephony service. This is a great opportunity to take on a role helping to ensure that we progress our strategic delivery plan and for the success and future development of the partnership
This is a fantastic chance for an Advanced Nurse to use their existing skills to develop their career in a new direction and fulfil a pivotal role in our service.
You will be required to provide a high-quality clinical service to the patients across the partnership, diagnosing and prescribing treatments and drugs, working within defined procedures and protocols as set out by the Partnership. The service will provide additional clinical resource across all of our 9 Branches. You will work from our Head Office in a purpose-built confidential area.
Clinically you will be responsible for receiving patients with undifferentiated and undiagnosed problems and assess their healthcare needs. You will carry out assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of the NSF’s, providing public health, health protection and promotion programmes that improve health and reduce inequalities. You will work closely with a team of GP’s and Practice Nurses.
Responsibilities:
Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
Diagnosing and treating patients presenting with acute medical and minor illness.
Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.
Provide assessment, screening treatment services and health education advice.
Provide general and specific health screenings to the partnership patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary.
Arrange for the collection of pathological specimens including intravenous blood samples, swabs, etc, as required.
Ensure the clinical computer system is kept up to date with accurate details recorded and amended.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Participate in the education and training of students of all disciplines and the induction of all members of the practice staff where appropriate.
We are looking for one or two (or even more) ANP’s to work a total of 37 hours a week Monday to Friday during normal surgery hours. We are open to being flexible with actual hours and days. The role will be based in our Head Office in Feering.
The post holder must be an Advanced Nurse Practitioner with experience of working within Primary Care and be able to demonstrate continued professional development and professionalism. They must hold a level 7 qualification in Clinical Assessment and be a prescriber.
The post holder must have experience of providing multi-skilled healthcare and clinical treatment and be able to work with patients and members of the public. They will have experience of working on clinical systems and be able to enter and use patient date efficiently, whilst maintaining confidentiality at all times.
The post holder must have strong interpersonal and communication skills and be able to liaise and communicate with clinical and administrative staff within each Practice and Partnership.
